Intensive Day Treatment Screening Process

  1. REFERRAL: To initiate a referral, call the Admissions Manager at 728-7500 ext. 143, or go to www.woodlandhills.org and complete the on-line referral form. The Admissions Manager will follow up with online referrals via telephone.
  2. INTERVIEW: An Intensive Day Treatment staff will contact the youth’s family to schedule a screening appointment within a week of the referral to determine if the Intensive Day Treatment program is appropriate to meet the treatment needs of the youth.
  3. MENTAL HEALTH DIAGNOSTIC ASSESSMENT: If a current assessment has been completed within the last six months, have it released and faxed to the Admissions Manager at 724-7403. If your client does not have a current assessment, you need to schedule one with a mental health provider. You may also have one done at Arrowhead Psychological. Time slots are available each week to schedule an assessment - please call 723-8153. Results of the assessment will then be released to Woodland Hills.
  4. PRIOR AUTHORIZATION: The “Pre-Placement Insurance Form” and the “Authorization for Use and Disclosure of Protected Health Information” will be faxed/mailed to the referring worker and/or the parent/guardian. This information is necessary to determine insurance benefits and to secure a prior authorization from the insurance carrier for services. It will also allow us to coordinate care with other service providers and assist in determining if there are alternative funding sources (other than insurance).
  5. ADMISSIONS: When (1) the youth has been interviewed and found appropriate, (2) a current diagnostic has been obtained, and (3) a prior authorization from the insurance carrier or another funding source has been secured, Woodland Hills will establish an intake date or explain in writing reasons for decline of placement with recommendations. This process typically takes from one to two weeks.
