> Make a Woodland Hills Day Treatment Program Referral

To make a referral to the Woodland Hills Day Treatment program, please follow these steps:

  1. Contact the Admissions Office (contact information to right)
  2. Download the IDT Referral Form (pdf | doc) and fax to Admissions Office
  3. Provide collateral information to include: assessment reports, social history, disposition report and discharge reports from previous placements
  4. In order to process the referral, the parent of a referred youth is asked to complete and forward the Pre-placement Insurance Form
  5. A copy of the current diagnostic assessment ~ not older than six months ~ is needed by Woodland Hills. If a current diagnostic is not available, the parent of the referred youth is asked to schedule one. A list of community providers can be accessed by calling the Admissions Office.
  6. Once a youth has been referred, the Admissions Office will contact the parent/guardian within one business day to schedule a screening interview for the child.
