Reports include: assessment reports, social histories, disposition reports and discharge reports from previous placements.
Intensive Day Treatment

3. The parent of a referred youth must complete and forward this information to Woodland Hills:
4. A diagnostic assessment not older than six months needs to be released to Woodland Hills. If a current diagnostic assessment is not available, the parent of a referred youth may schedule one by contacting Jonathon Beyer, PhD, LP of the Arrowhead Psychological Clinic (218.723.8153).
Time slots are reserved for diagnostic assessments for Intensive Day Treatment referrals.
5. Once a youth has been referred, an Intensive Day Treatment staff person will contact the parent to schedule a screening interview.
For referrals to the Department of Corrections State Commit Program (female):
1. Prior to a commitment to the commissioner being made, the probation agent needs to contact Kyiunga Olson, Minnesota Department of Corrections juvenile female intake worker, at (651) 267-3667 to determine the offender’s eligibility. Commitment criteria
2. If the offender meets the commitment criteria the probation agent should then contact Woodland Hills to complete a referral. Please contact the Admissions Coordinator, Glenn Dallmann:

For referrals from outside Minnesota:
1. Please contact the Admissions Coordinator, Glenn Dallmann:

3. Secure an Interstate Compact Agreement with your state's interstate office.
4. Complete a tuition agreement form, which the Woodland Hills’ Admissions Coordinator will provide.
